LVA ( ) and Agency 1 agree to the following roles and responsibilities regarding the management of the ______________ Project from _________________ to ___________________.

LVA ( ) will:

• hire and supervise a project facilitator
• provide training for the project facilitator and related materials
• provide an office for the project facilitator
• work with the project facilitator to develop the program model and to write the interim and final project reports
• manage project funds
• interact with the main funding organization
• purchase $2,000 of books and materials for the project
• provide 10 trained volunteers to conduct instructional sessions

Agency 1 will:

• provide copying, printing, and on-line services to the project
• provide administrative support one-half day a week
• provide classroom space for the project participants
• recruit 20 participants from its service population
• purchase $3,000 of software for the project
• assist in development of the program model based on its knowledge of population needs and goals

Both LVA ()and Agency 1 will:

• interview the project facilitator
• supervise the quality and conduct of the project including activities and timelines
• evaluate the project as well as the success of the partnership
• meet on a monthly basis to address new directions, obstacles, and successes
